Update utility scripts and docs for Bionic.

 * Update refresh-deb-repo and related scripts to
   use bionic, and remove unsupported platforms.
 * Update LXD container creation scripts to use
   bionic instead of xenial.
 * Update LXD container creation scripts to assume
   a git-based environment with no bzr shared repository.
 * Update documentation to reference bionic as the
   supported platform when installing MAAS.
 * Remove obsolete isolated-make-test script.
   (This script depended on bzr sandbox layout.)
