Fix invalid element in adaptivity_options schema

Relax-NG doesn't allow mixing string and data in a single group in an
element. Fix this in the adaptivity_options schema and update
Adapt_State_Prescribed for the new options path: /mesh/from_file/name
is now /mesh/from_file/format/name.

Apparently ar -q does add new object files into the archive if they alreadt exist there on some systems (i.e. Ubuntu). This means when you chnage existing code it doesn't get built into the binary. Switching to -r
