Merging the les-extend-and-fix branch into the trunk. Thanks to Alex for the review.

Main changes:

- Added the 'scalar'/Deardorff length scale from Jon Bull's turbulence branch.
- Added a unit test for the 'scalar' length scale.
- Renamed LES_viscosity.F90 to LES.F90 since it contains subroutines that do things other than calculate the eddy viscosity tensor.
- Removed the assumption that the mass matrix can always be lumped when solving for the LES diagnostic fields. Users now have the option to either lump the mass or do a consistent solve.
- Rearranged/renamed various subroutines for computing the LES diagnostic fields to try and make it a bit clearer for users and developers.
- Added an MMS test for the second order model, using the P2-P1 element pair and the 'scalar' length scale. This also tests the computation of the EddyViscosity diagnostic field.
- Added a 2D validation test case of flow past a "square cylinder". This includes a k-epsilon version for comparison. This test will be moved to the longtests directory after merging.
- Added documentation to the code, schema, and manual.
