Remove test cases that were added in this branch that do not work yet - some tidying.

Remove flux BC from consideration of CV pressure BC - not sure if it is needed.

Better comment the time term coefficient and test continuity with CV options.

This branch needs more tidying then breaking up into:

- adding time term coefficient CV, CG, DG, FV (code, test, doc)
- fix weak vel BC with pressure CV (plus some darcy tests)
- pressure CV bc + sum velocity divergence (code, test, doc)
- for cg pressure test continuity with CV (code, test, doc)

Evaluate pressure CV boundary conditions with the velocity theta
weighting. Also for multiphase if the phase volume fractions are all
CV then form the surface values differently taking account of upwind
face value due to it possibly having a weak BC also. Also evaluate these
terms with the velocity theta weighting.

Darcy pressure BC test cases still pass.

A first attempt at a compressible test case with CG pressure with
CV weighted continuity. This is derived from the shocktube_1d test
case with the velocity check removed as the scripts didnt work for dg.

The solution (pressure, density and internal energy) appears to stick
to the left boundary like a dirichlet BC is applied, when it should actually
change.
