feat: add TLS/SSL support to PostgreSQL database connections

Add sslmode configuration (disable, allow, prefer, require, verify-ca,
verify-full) with appropriate TLS handling for Temporal migrations, UNIX
socket special handling (no SSL parameter), and backward-compatible
defaults. Includes 5 focused security-critical tests.
