Do not export all pools

Only export pools that contain datasets mounted under /target

Use persistent path for pool's vdevs

Devices names such a /dev/sdX may change on reboot and lead to
unbootable systems.
This patch uses partuuid instead which is stable. We couldn't use uuid
because it changes when the pool is created. (LP: #1880869)

make grub_default consider the boot argument in the non-removable case

grub_default got a boot= argument way back in 2012:

   https://git.launchpad.net/ubiquity/commit/?id=989b413b8a4b8f785d4d2e85998deff59a90c5cf

but it only really handles the case that Ubuntu is being installed onto
a removable drive. Update it to return its argument if it is one of the
devices grub-mkdevicemap returns.
