Merge smtp-connection-errors

Add transient error handling for ConnectionLost and ConnectionDone
and generalize the ConnectionRefusedError handling to all
ConnectErrors.

This prevents transient errors such as a route being unavailable
or a connection being closed prematurely from permanently failing
delivery and instead causes the delivery to be retried.
