Summary: Use better bookkeeping to prevent needless duplication of effort:

  - On initial startup, find the most recent bookmark (by its last
    modification date), and remember that for future Firefox
    launches. We only need to push changes that are newer than we've
    seen before.

  - When pulling bookmark changes, remember the last known sequence in
    Couch across launches, so we only need to pull newer records from
    Couch.
