Summary: Use better bookkeeping to prevent needless duplication of effort:

  - On initial startup, find the most recent bookmark (by its last
    modification date), and remember that for future Firefox
    launches. We only need to push changes that are newer than we've
    seen before.

  - When pulling bookmark changes, remember the last known sequence in
    Couch across launches, so we only need to pull newer records from
    Couch.

This branch store remotely-created bookmarks in a folder in the bookmarks menu called "Desktop Couch" when the remote bookmark doesn't explicitly live in either the toolbar or the bookmarks menu.

This branch fixed #443121 by avoiding the replication of livemarks and dynamic folders. It also keeps notifications about bookmarks in those folders from being propagated to CouchDB.
