Cleanup to work with modern glibc - Don't include custom glibc
headers, make sure all needed glibc headers are actually included.

Do not include linux/ip_icmp.h, include the relevent segment from it.

* Build system cleanup. Stop including anything from /usr/src/linux.
  We still define our own versions of things that should be included
  from /usr/src/sys, and this needs to be fixed, but I will wait until
