Cross model relation support

Cross model relations are anonymized, so publishing authorized
unit names on the relation will not work. Instead, publish
the list of autorized subnets which clients can match with
their egress-subnets to see if they have been authorized.

allowed-units is deprecated, but will remain supported for the
forseeable future for single model deployments.

v2 protocol

Each PostgreSQL unit will present the master connection string
and all standby connection strings. This change is mainly
to support proxies when the number of proxy units does not
match the number of PostgreSQL units, such as a a single
pgbouncer unit proxying a replicated PostgreSQL service.

Charms should prefer the 'master' and 'standbys' connection
strings over the old individual components, which will allow
us to specify more advanced connection options such as sslmode.
