Bugfixes:
- declare xapian document in the main loop of index_menu_item, otherwise it's reused and leads to incorrect results.
- use g_app_info_get_commandline instead of g_app_info_get_executable, since the latter returns only part of the commandline (e.g. 'gksudo' instead of 'gksudo xdiagnose')
- break the switch statement on unexpected item (i.e. continue main loop) instead of returning from the index_menu_item method.
