Significant cleanup of libspud

* Fortran to C now done the official way with iso_c_binding
* Several old Fortran compatibility hacks removed from the C interface.
* Interfaces made more consistent so that essentially all routines return error states.
* Header files renamed to more conventional names.
* C routines renamed to spud_foo instead of cspud_foo.
* The manual updated accordingly.
