build initramfs with MODULES=netboot, install mlx4.conf file if not present.

This gets the initramfs built with more network modules, which is clearly
important since this is a network boot initramfs.

The other thing it does is a workaround for the mlx4 driver to get it loaded
correctly. That will be fixed in distro at a later date.

as of today, the archive proper archive contains everything necessary for
12.10. So only add the ppa for 12.04.

Instead of removing entirely, we only comment it out.
The reason for this is that we do not want the ephemeral image to
expect to be able to access ppa.launchpad.net.
