Fix URLs of git repositories (ownership changed)

The ownership of the git repos for the ubuntu-docker-images project
has recently changed, so we have to adjust the URLs here as well.

Add 21.10 information for all images (and a few other adjustments)

- This commit adds information regarding the 21.10 releases for all
  images. We were doing that back when the YAML files were split into
  their own repos, but I think something might have been lost during
  the migration.

- I took the liberty to make a few adjustments here and there on the
  styling/formatting.

- I've also added version information for all images. This was
  missing from a few tracks.

Delete unused 'data/' directory

We've been keeping each image's YAML data (used to generate the
templates) inside their own repositories. Our 'data/' directory is a
relic from when we used to keep everything in this repo. This commit
gets rid of it, and also updates the HACKING_docs.md template to make
it clearer where the data is located.
