rewrote everything to support 3D, including gen_polyhedron_surfmesh. There is an issue with testcase #3 (mesh_metric2_example.py), when running with debugging

fixed integer wrap-around for surface mesh generator, removed input mesh from metric_pnorm, added convergence test for maximal_example - way nicer than circle_convergence (now renamed to ellipse_convergence), added support for CG1 mesh in metric_ellipse (for metric convection), added logexpmetric and logproject as alternatives to fix_CG1_metric(, which takes the absolute value of the eigenvalues)
