Use the new @subentry for subentries

We previously used a hack to get multi-level indices. Texinfo 6.6dev
now supports this via @subentry, so have the macros expand out to use
@subntry.

Include makeinfo 6.6dev

For now we need our own copy of the 6.6dev version of makeinfo (and
txiversion.tex) to get the latest features in 6.6dev.

Update texinfo to 6.6dev (May 18, 2019)

Update texinfo to the 6.6dev version that supports @subentry for
creating multi-level indices that we use for the category indices.

Maybe this can also be used for the category indices in the info
version and html version? That would probably change the html
version, but it might be consistent with the pdf version, then.

translator: Remove (PROGN 'COMPILE ...) flattening from SUB-PRINT*

The translator no longer generates (PROGN 'COMPILE ...) forms, so
let's cut out this flattening effect from SUB-PRINT*.

See also commits c3a3b695 and 4c5d6b45.

translator: Generate (PROGN ...) instead of (PROGN 'COMPILE ...)

The translator generated other (PROGN 'COMPILE ...) forms in the
past, but they have since been changed to (PROGN ...).

See also commit c3a3b695 for more on (PROGN 'COMPILE ...).

c3a3b69... by Kris Katterjohn on 2019-05-18

Replace (PROGN 'COMPILE ...) with (PROGN ...)

In Maclisp and Franz Lisp the quoted COMPILE in (PROGN 'COMPILE ...)
was required at top level to tell the compiler to actually compile
the subforms as top level forms. Treating the subforms as top level
forms in this case is standard Common Lisp, so let's get rid of the
quoted symbol that doesn't do anything useful anymore.

There were many more of these (PROGN 'COMPILE ...) forms in the past,
but they have since been changed or removed.

Share testsuite: Marked a few tests as "known good"

Many "known bad" tests were bad in Ubuntu Trusty - which has
now exceeded its support livespan except for a few paying
customers. I guess this is the Right moment to start marking
tests that werde known as bad with a linux that appeared in
2014 as "known good" if they work with a current linux
version.

Updated the wxMaxima version.

This time the version update is to incorporate all the
results from our testers.
