 - Added to explain more about the plugin, including details about its installation.

 - Move some information from header comments to the README file.
 - Add new options $modeline, $rst2html_options
 - Do not use of wptexturizer; its latest incarnations play around too much. Let the reStructuredText engine dictate the output.
 - Make the more-text search/replace more template-agnostic.
