Migration ceph stable charms to charmhub - octopus

This is the stable/octopus branch of the ceph charms. It tests against
focal-ussuri and bionic-ussuri (validates octopus).

As a minor fix-up, it also adds a versioned upstream zuul job
to handle check and gate pipelines without breaking on future releases.

Use openstack-charmers namespace when deploying

The charm will not deploy without specifying the
'openstack-charmers' namespace as the charm is not
promulgated at this time.

It would be good for this change to be backported.

Improve README

Fix exception when chain is missing

Handle the situation where a chain file has not been passed along
the certificates relation.

Update README deployment instructions

The stable charm is currently in tech-preview and so is not promulgated
to the cs:ceph-dashboard. During the next cycle the charms will move to
charmhub (with a different system), so this change is not being done to
master first. Thus for this stable set, the deployment instructions
need to call out cs:~openstack-charmers/ceph-dashboard.

Add note about Grafana certificate

Some environments may experience failure as Grafana graphs
will silently fail to display due to an unverified TLS
certificate for the Grafana server. Add a note about this
and how to overcome it.
