The DB indexes could never be used because we did case-insensitive comparisons
in queries.

This merge fixes that, using the correct case-insensitive collations in DBMSs
supporting them, and CITEXT in PostgreSQL.

That required some some more Sqlalchemy wrapper code and custom types.

Changed the multiline behaviour in the bot, to conflate multiline responses on
non-multiline-supporting sources, unless flagged with conflate=False.
In that case, the response is split into multiple responses.

All response pre-formatting now happens in a core plugir rather than the
dispatcher.

Mark an event as processed as soon as an exception is detected.
This will stop another processor (such as regex-factoids) from returning an
unexpected response.
