Hooks: allow only one plugin per manifest

Each plugin has its own manifest file, so there is no need to allow more than
one plugin per manifest. This also simplifies the ID assigned to the providers.

Hide providers which no application can use

Filter the provider list to include only those providers which can be used by installed applications. If there are no applications which can use a certain account, hide the relative provider from the list of creatable accounts. Fixes: #1495531
