Changed headertests option in CMakeLists.txt to slowtests option, and added IsolatedScope test to slowtests, so we don't have to wait 40+ seconds each time we run the tests.

Added and fixed lots of doc comments. Added tutorial.dox to list of
dependencies for doc target. Removed create_scope_runtime() from
public API because that method is only for internal use.
Removed warning summary from CMakeLists.txt because there are no
more warnings. Going forward, we want to see any warnings from
doxygen, seeing that the API is mostly stable now.
