Simplify default mapper urls.

Abolish default_help/default_handler and instead rework the url configuration
to let app users choose what they wish to import. You either map the whole
urlpattern to get the default behavior or mount tokens and handler+help
separately.

Each invalid request (GET or empty POST) gets redirected, with a temporary
redirect, to a API help page.
