don't declare and then define, but do it all at once. It's faster and it
doesn't leave the variable in a null state for a while.

Fix addition and removal of elements

Removal was relying on selected item, which ended up making it hard to
keep removing one after another. Also it was checking on redundant
properties like the model being empty.
Addition was not working if the model is empty or if there's no selection,
which doesn't make sense.
