Add key_extras to proxy authentication schemes.

The key_extras value is a "quoted string" with logformat %macro support. It is
appended to request line for the authentication helper.

Example usage:
   auth_param basic key_extras "thePort:%>lp"
   auth_param digest key_extras "LocalIP=%>la:%lp"
