add "region roll over"

In the case where an attempt to launch an instance returns an
   InsufficientCapacity
or
   InstanceLimitExceeded
Roll over to the next region in the settings.REGIONS_TRY_ORDER

This could be enhanced in the future to not attempt to launch in a
region where there the database had N instances running where N
was greater N (settings.REGION_LIMIT['region'] or something)
