Updating install and created docs file

Moved file out of install to docs as its marked 'sample' and I figure thats
a clue....
The other file is now installed explicitly into the root of the php directory
since it seems pointless having a directory for one file.

uscan still isn't working, but when it does this comment will probably be
applicable - unless uscan is happy to download files without extension.

AFAIK these files are non free, so need to be in that section. Link below is
completely proprietary and the files have no header to indicate their licence

Finish packaging for module

This has a known and deliberate error: installs to usr/share/php-payment-express.
Not sure where files like this should end up so that will do 'for now'.

More common changes

I just noticed dh-make-php and dh-make-pear were being added as dependencies
but arn't needed in these packages. The ones I've already uploaded will have
to be pulled down and adjusted but for now they will stay as is.

Update template where the values are identical accross packages

I've just wised up - instead of doing this every time I can take a copy of
this debian directory and copy it in to each new package I create.

Next commit will be package specific.
