Enhance watch file and add get-orig-source d/rules target

the enhancements come from the debian wiki which has a nice little snippet
to use for github which even handles the file name mangling.

More common changes

I just noticed dh-make-php and dh-make-pear were being added as dependencies
but arn't needed in these packages. The ones I've already uploaded will have
to be pulled down and adjusted but for now they will stay as is.

Update template where the values are identical accross packages

I've just wised up - instead of doing this every time I can take a copy of
this debian directory and copy it in to each new package I create.

Next commit will be package specific.

Clean Debian directory using dh-make-pear template
