Getting some compile errors -- something about incomplete
type used in named namespace specifier. Probably an
include file location needs fixing, but gonna get Monty
to take a look at it...

* Renamed some files to prevent include conflicts
* Added include guard on drizzled/item.h
* Continued cleanup of item.h and item.cc
* Added mysql_priv.h include protection in item/* headers to allow proper lib building
* Corrected stoppid copy/paste mistake which renamed MY_INT32_NUM_DECIMAL_DIGITS to MY_INT32_INT_DECIMAL_DIGITS
* Update Makefile.am in item/ to reflect filename changes

* Renamed item/float.h/cc to item/ifloat.h/cc for obvious reasons. Not
  sure why I forgot about that one.

* Updated item/Makefile.am to new item/ifloat.h/cc

* Some cleanup in item.h and item/int.h
