Refactored code to make it easier to test it

Preseed.read/write renamed to Preseed.load/write (same method names as in the
yaml library). After all, the filename wasn't an important part of the class
itself and using strings for input/output makes easier to verify the
implementation.

Added DuplicatedQuestionName exception

When a duplicated question name is found when parsing a question name, a
DuplicatedQuestionName exception is raised with the question name that has been
found as duplicated. This should make easier to debug problems in which the
preseed file has such a problem, since the old assertion error wasn't clear
enough.
