Created a MergeableList class and used it for phone_numbers.

- Added phone_numbers and urls. Noticed when I came to do the other
  mergeable list attributes that this is going to have to be reworked.
- Added notes, company, department and job_title.
- Fixed generateMergeableList():
  - We now don't end up with null UUIDs in _order if the card value
    doesn't exist.
  - Ditto if the UUID attribute doesn't yet exist (because this is a
    new value).
  - Respect the description value that's stored in the card attributes
    if it exists, as the user may have changed this from our default.

Added generateMergeableList to deal with all the, um, mergeable lists.
Added email address push using the above.

Commented out the email_addresses part as it breaks Ubuntu One.
Set the document ID using the contact Display Name.

Modified some method signatures to get card properties working.
Stubbed out a Mergeablelist for e-mail addresses. This is a hack.
