Remove default RDF metadata

- Remove hard coded default metadata
- Remove metadata from default.svg
- When writing document metadata, only search in root for
  <svg:metadata>, do not recurse into child nodes which could have their
  own metadata.
- When clearing the RDF title (with document properties dialog), also
  clear <svg:title> (make synching consistent)

Fix rebased URLs after running extensions

Bug is only observed if `$TMPDIR` is not at the same level as the
document's location, and relative links are possible (common ancestor is
not the filesystem root).
