Fix the handling of disconnection errors in the storm.django bridge.

Django was replacing the exceptions from the database adapter with its
own, which broke the disconnection handling. Furthermore, in the case
of a disconnection, the store was sometimes not correctly registered
with the transaction manager, which would prevent the store from being
rolled back and reconnected on a new transaction.
