Fix pvtu2vtu for DG pvtus.

Previously when passing a discontinuous pvtu to pvtu2vtu, it would create a continuous vtu
choosing a value at the vertex from one of the surrounding dg nodes at random. Now, it creates
a discontinuous vtu without loosing any information or accuracy.

1. Lifted equation_type_index (which makes expensive calls to spud) outside an element loop. This made the RANS test problem I was looking at run 2-3 times faster.

This test depends on having spudtools installed. Spudtools is installed before medium tests, by default, so changing test from short to medium on the advice of the PHENOMENAL MAN that is the test owner.
