Fix a bug that would break maps for types with external use specification

Add tests that would fail before this fix. Add a couple paragraphs in
documentation to stress that types with external use specification
must have explicit assignment operator.

Cherrypicked bug-fix from the trunk

Trunk revision 154.

Before, a table could be left with too many occupied buckets. Due to
the way lookup_or_insert() is implemented, this wouldn't lead to any
real troubles though.
