fix rabbit-worker upgrade

Made a couple of dumb mistakes with install_from_tarball

Also noticed that we should simply run "update" instead of
configure_source during an upgrade opertaion. configure_sources
will raise a fatal error if apt-get update fails, which in many
cases is okay for us.

This adds the "--upgrade" flag so an existing deployment may
have its config settings and code upgraded.

image-builder: support for configurable apt mirror

Based on suggestions from:
 http://blog.utlemming.org/2013/04/official-ubuntu-mirrors-in-hp-cloud.html

I've updated the sources.list file we build for test images to optionally
include the cloud mirror.

NOTE: these images can still run outside of a cloud as well, but when
run in the cloud will be faster for downloading/installing packages.
