Add '-partition_offset 16' to xorriso parameters

Suggested by Thomas Schmitt, who describes the following benefits:

[-partition_offset 16] will yield a more conventional start of partition 1

but also will enable xorriso >= 1.4.8 to let the ISO superblock of
the device claim the range up to the end of partition 2.
So commands like `isosize` will be in sync with the image file

Point to the el torito catalog to the appended partition

This will allow us to remove efi.img from the ISO filesystem and recover
that space. That commit, however, does not perform that removal.

Drop xorriso parameters that have no effect

Passing "-boot-load-size 4" and "-boot-info-table" before "-eltorito-alt-boot"
has no effect. It is likely cruft cargo-culted from x86, just drop it.
