Drop icehouse from deploy from source defaults

The default git repos don't work as-is for deploying icehouse so let's not
allow them to be deployed via default openstack-origin-git values. The reason
the default git repos don't work is because icehoue didn't have the concept
of upper-constraints so the depenencies aren't capped, which causes problems.

Partial support for kilo-eol tag

Update the default deploy from source branches since the core repos
have moved their stable/kilo branches to the kilo-eol tag. The
requirements repo is still using the stable/kilo branch.

Clone/install nova git repo on neutron-gateway

Ensure that nova git repository is cloned and installed on neutron-gateway
when deploying openstack from source using the default openstack-origin-git
config values. This is required to run nova-metadata-api.

The upper-constraints.txt file from the openstack requirements git
repository defines known-good dependency versions. Use this when
installing with pip to ensure known-good versions are installed.
