Enable OpenStack Mitaka version of swift.

OpenStack Mitaka and Liberty both currently have the same version of
swift, 2.5.0. The SWIFT_CODENAMES dictionary and code that uses it are
rewritten in this commit to support more than one OpenStack release
containing the same package version.
