window-menu-model: create a GtkMenuBar for window models

This doesn't have any impact other than that gtk_menu_bar_new_from_model
doesn't insert GtkSeparatorMenuItems between elements. These separators were
turned into empty (invisible) IndicatorObjectEntries which interfere with UI
testing.

This also saves N/2 indicator entries from being needlessly created. Fixes: https://bugs.launchpad.net/bugs/1089997.
