 - Several minor fixes to comments and things.
 - add_group_depths() now adds '_depth' fields, not 'depth' fields.
 - construct_themed_user_link() renamed back to underscore form. This is
   because this function belongs in a general-purpose core API file, not
   this one, so it isn't guaranteed to not be removed from this file.

User groups list:
  - 'My account' title changed to username. Now matches other pages
    (Drupal overrides the string in this way too).
  - Removed check on $cug_own_groups_list and always show non-'Your'
    versions of title strings.
