Fix a bug where multiple notices entries could end up in the database. When a new notice was downloaded, we added a new version instead of updating the existing version.

Better test coverage.

We don't need square brackets around the Message-ID since it will almost
always have angle brackets anyway.

- Entry.key -> Entry.context
- Ignore message if it has a List-ID header
- Add an X-Bug-Tracker header to responses
- Better explanation of reply_context in the sample cfg file
