Call fork() before gtk_init().
Calling fork() after gtk_init() can have side-effects such as breaking DBus
in the child if gtk_init() opened a DBus connection. One such situation in
which gtk_init() will open a DBus connection is when Ubuntu menu proxies or

Correctly support program renaming
The autostart desktop file is also renamed. This is provided for distributors
who want to use xfce4-volumed-pulse as a drop-in replacement of xfce4-volumed

Do not show notifications when nothing changed
Pulse seems to emit a sink updated signal when a new sound program starts (e.g.
when the browser loads a flash video). Don't display a notification in this
case.
