Ambiance, Radiance: unify the .titlebar and UnityDecoration definition

Use the UnityDecoration style for default gnome titlebars and use just one
definition for both. Don't use the primary-toolbar style for headerbars
unless they're in toolbar mode (only in unity, then).

Ambiance, Radiance: don't threat any headerbar as primary-toolbar

This is just wrong as there are headerbar that are not titlebar around (i.e. gedit),
while this might be true in Unity, the app should be properly patched to export this.
Side effects of this are tracked in LP: #1689239
