* cherry-pick upstream fixes
  - bounce: segv on one-line template text.
  - sasl: enforce mechanism output filter on client command input.
  - milter-app callbacks didn't receive {mail_addr} or {rcpt_addr}

  - qmgr was incorrectly initializing client logging attributes

    auth failure.
