You do not have to restore anything. You should have my proposed pdftopdf and pdftops filters and Ghostscript 8.64.

Check whether Ghostscript works correctly running

gs -h
ldd /usr/bin/gs

These commands should not show any errors.

Try also running the command lines from

http://bugs.ghostscript.com/show_bug.cgi?id=690236

using combined.pdf as input file.
