Avoid glitch when changing keyboard layout

The keyboard layout combobox used to refresh its content every time it is
changed. This breaks manipulating the combobox with the arrow keys. With this
patch the keyboard layout and keyboard variant comboboxes are only updated if
the new content differs.

Moving question dialog to use a glade ui file, made it non
"re-entrant" safe, i.e. it started to remember previous action area
buttons. The fix is to clear action area, all other parts of the
dialog are always reset with something fresh. (LP: #1087823)

Revert change from rev 380 to prevent memory leak

Starting around the time rev 380 was introduced, we've been fielding X
crash reports, where the system appears to run into an out-of-memory
condition. (X crashes have generally been due to trying to instantiate
graphics but cmalloc() calls failing and returning NULL, then those
pointers being used unchecked.) So, on the X end we've been fixing
symptoms but not the root cause.

Robert Hooker finds that with this ubiquity commit reverted he is not
able to reproduce the issues.
