To get what we want, verbatim lines must lead with a tab regardless of how they are otherwise indented. This seems to be a weirdness of Pod::SAX, and I'm starting to think a little customization of that class may be in order (I've already had to manually convert an HTML4 entity as well, and I disagree with its handling of S<> even though I don't actually use that tag ever).

Their implementation is straightforward enough that I don't feel terrible implementing the extension... Not yet tested. * ustrings.xsl is an XSLT sheet that cleans up the interface to the extensions in UStrings.pm. Not yet tested. * htmlify.pl: Now loads UStrings.pm. * to-html.xsl: Now loads ustring.xsl.
