Tweak debootstrap handling:

The correct path for debootstrap code is /usr/share, not /usr/lib; use
that and assume it now.

Cope with the $(CODENAME) script not existing, and create a symlink to
sid where necessary. Just found in jessie - debootstrap needs
updating.

Standardize on 30 seconds timeout for server images booted in UEFI or BIOS mode. This is no change for live images that do no use hidden-timeout, they already had a timeout of 30 seconds.

Add '-partition_offset 16' to xorriso parameters

Suggested by Thomas Schmitt, who describes the following benefits:

[-partition_offset 16] will yield a more conventional start of partition 1

  test.iso2 332712 334759 2048 1M ef EFI (FAT-12/16/32)

but also will enable xorriso >= 1.4.8 to let the ISO superblock of
the device claim the range up to the end of partition 2.
So commands like `isosize` will be in sync with the image file
