Thanks for looking into this. I suspect there may be a couple more causes of the error though, as I can still reproduce it with the fix. I've been testing it on my OS X machine at home and get the same errors as I was seeing on Windows at work. The attached shell script does 100 commits concurrently on two branches, and the commits fail intermittently with the NoSuchFile error. There seem to be at least three different variants: the one that's now fixed (see traceback above), a similar one with the the index files, and a different one while renaming the pack files. I'll include the tracebacks below.
