it's visible in update-manager if you click on one of the buttons which opens an another software

other cases when it happens:
* start a software and click somewhere else before it starts, the focus stealing prevention will trigger and the decoration have the squares
* use the run dialog and type until it finds a match and displays an icon in the dialog
